To Register as a New Patient

To register with Claudy Health Centre, you will need to complete the appropriate forms which is available at reception and make an appointment with a GP.  We will need details of your name, address and current GP to assist in the prompt transfer of your medical records.  You will be asked to provide photo ID and proof of your current address.

We will not be able to offer you an appointment to see a doctor or nurse before you have registered.

Temporary Residents

If you are not registered at Claudy Health Centre, but feel you require emergency treatment, contact reception for further information.

Change of Personal Details

It is important to let us know of any change of name, address or telephone number, in order to enable your records to be kept up to date.  The relevant forms are available at reception.
